Job summary

A leading healthcare provider in the UK is seeking a dedicated individual to assist in delivering patient centred care under the guidance of a Consultant Social Worker. The role involves collaborating with various stakeholders, providing practical support to service users, and working within a multi-disciplinary team on a 7-day, 24-hour service basis. Job description
Responsibilities

You will be responsible for assisting with the delivery of patient centred care under the supervision of a social worker, providing practical support to service users in performing day to day activities, guidance and education, and in assisting in their management and care based upon individual needs under the guidance of a senior member of the Team.

You will be expected to work collaboratively with key partner organisations, service users, carers, clinicians and other practitioners within the multi‑disciplinary team in delivering services; providing a 7‑day a week, 24‑hour a day service for 365 days a year, working shifts where appropriate and operationally required.

In delivering your duties you are expected to display behaviours that are aligned with the Trust values and as required in accordance with the NHS Constitution at all times.

The successful candidate will work with carers and families of people with mental health needs under the direction of a Consultant Social Worker. You will support and enable the carers to connect with local assets within the communities to help build resilience.
