Qualifications

• Experience of using computer and IT systems with a basic level of IT literacy.

• Willingness to undertake relevant training by the Personalised Care Institute.

Responsibilities

• Identify patients who meet specific criteria using population health intelligence.
• Deliver personalised holistic healthcare and support to manage patient needs.
• Support patients to access self-management education courses and other interventions.
