A leading healthcare provider in the UK is seeking individuals for their Cardiac Rehabilitation Service to assess and treat patients post-event. This role involves working closely with a multidisciplinary team and providing specialist advice to ensure optimal patient care. Candidates should possess excellent communication skills and the ability to work autonomously. The Cardiac Rehabilitation Service provides care, support, education and exercise options following a cardiac event such as MI, PCI and cardiac surgery. You will work alongside the multi disciplinary team to help provide this. You will ideally have some cardiac nursing experience, however, full training and support will be provided. Work autonomously within Trust policies and professional practice, maintaining accurate and comprehensive up to date records.
To carry out assessment and treatment of cardiac patients from inpatient stay through to full rehabilitation in the community with support and guidance from senior colleagues.
Provide specialist advice, ensuring appropriate pathways of care and communication via liaison and referral to other agencies as required.
To support the Team in the day to day running of the Cardiac Rehabilitation Service.
To support and advise within the wider MDT on the delivery of the service.
