An NHS Trust in the UK is seeking a full-time Cancer Pathway Navigator responsible for coordinating patient care within the Lower GI cancer pathway. The successful candidate will ensure timely treatment and care, using multiple IT systems to track patient progress and communicate with both clinical staff and patients effectively. The role requires excellent communication skills, IT literacy, knowledge of national cancer targets, and any relevant experience in a healthcare setting.
TheColorectal Department has an exciting opportunity for a full time CancerPathway Navigator. This post is crucial in ensuring that patients are seenand treated as part of the Lower GI cancer pathway in a timely fashion. Thesuccessful candidate must be proactive and have excellent communicationskills, being able to liaise with patients and both clinical andadministrative staff.
Youwill require a good knowledge of national cancer targets and be confident inapplying these guidelines to a patient pathway. You must be IT literate andable to use several different IT systems to perform this role to the higheststandard.

Thepost holder will
Communicate with relevantdepartments and organisations to ensure that the patients pathway is astimely as possible, and all delays are avoided.
Communicate with key departments(e.g., diagnostics, clinic bookings) to ensure all tests and investigationsare completed in a timely manner and any issues are escalated.
Communicate information on thepatients progress on the care pathway to CSU managers and clinicians asappropriate.
Liaise and communicate cancerguidance and definitions to key staff within the Trust.
Communicate regularly with patientsto ensure they are aware of upcoming appointments, tests, investigations, oroperations, and offer practical support with the arrangements.
Develop positive workingrelationships with all members of the team, liaising in particular withmanagement, Consultants, specialist nurse, and administrative staff.
Manage own workload, with theability to work independently and take initiative where appropriate; planningand organising their own time efficiently and effectively to meet allrequirements.

Thepost holder will work both directly and indirectly with patients and theircarers, using a range of IT systems to assist in coordinating patient carethroughout their hospital journey.
Thepost holder will coordinate appropriate services to maximise the delivery ofstreamlined diagnostic pathways, thereby improving quality of care andexperience for patients and their carers.
Thepost holder will work across management teams and organisational boundaries,whilst linking in with the Cancer Centre, and contribute to plans anddevelopments.
Thepost holder will be responsible for the review and updating of the Lower GItracking spreadsheet which informs the cancer pathway. They will also beresponsible for ensuring that any required actions take place.
