A healthcare provider in Derbyshire is seeking a compassionate professional for their Triage and Assessment team. This role involves triaging young people to ensure proper pathways, conducting mental health assessments, and working with a multi-disciplinary team. You will be responsible for carrying our triage calls to all young people on our assessment wait list to ensure they are on the correct pathway.
You will then carry out robust mental health assessments, formulating a plan of care, risk assessing and liaising with other teams internally and externally to CAMHS. You will have regular contact with young people suffering from moderate to severe and enduring mental health difficulties from a wide range of backgrounds.
You will be part of a multi-disciplinary team and hold a small caseload of young people presenting with risky behaviours.

The cost of undertaking a DBS (Disclosure and Barring Service) disclosure will be met by the individual. The Trust will meet the initial costs of the disclosure which will be deducted from your first month's salary.
