Role: Ward Clinical Pharmacist
Banding : Band 8a
Location: Westcliff-on-sea
Start Date: ASAP
Working Pattern : Monday to Friday, 09:00 – 17:30pm
Rate: £34 - £39 per hour

We are seeking a highly skilled and experienced Ward-based Clinical Pharmacist to join our team in Westcliff-on-Sea. The successful candidate will provide comprehensive clinical pharmacy services on the wards, ensuring the safe and effective use of medications. This role is essential in maintaining high standards of patient care and supporting the multidisciplinary team in delivering excellent service.

Duties and Responsibilities:

In this role, you will be responsible for providing clinical pharmacy services on the wards, including medication reconciliation, clinical screening, and patient counselling. You will work closely with the healthcare team to optimize medication therapy, manage discharge medications, and ensure seamless transitions of care. Your role will also involve participating in ward rounds, supporting clinical audits, and contributing to quality improvement initiatives within the pharmacy department.
