We are currently looking for an experienced Advanced Clinical Pharmacist , who can provide overall support for the clinical leadership of the service. This post is based in Boston, which is easily accessible via public transport.

About the role :

As anAdvanced Clinical Pharmacist, you will beworking in a clinical and service development capacity to ensure the service to patients is planned, delivered and evaluated in line with evidence-based practice.

Your main responsibilities as anAdvanced Clinical Pharmacist :
  • Provide specialist clinical pharmacist support to RDFC and provide support to GP Practices as appropriate, as per objectives set between the PCN Manager and Practice Manager
  • Exercises professional supervision over dispensing activities for inpatients and outpatients
  • Support a team of clinicians within GP Practice, taking responsibility for areas of chronic disease management
  • Improve value and outcomes from medicines across both primary and secondary care sectors, consulting with and treating patients directly
  • Manage long-term conditions and provide care and advice for patients with complex poly pharmacy needs
  • Manage patients who are at risk of admission / re-admission to hospital and ensure patient safety
What we are looking for in an Advanced Clinical Pharmacist :
  • Master’s degree in Pharmacy
  • Registration as a Pharmacist with General Pharmaceutical Council
  • Supervisory experience
  • Independent prescriber qualification
  • Medicines Management experience gained in a Hospital environment across a range of specialities
