A mental health service provider in Bristol is seeking a professional to support young people experiencing mental health challenges as they transition from children's to adult services. The ideal candidate will hold relevant qualifications, have experience in community mental health services, and be proficient in providing specialist advice to stakeholders. This part-time position entails a band 7 pay scale, offering £47,810 to £54,710 annually on a fixed-term contract. The role is crucial in ensuring a smooth transition and long-term recovery for young adults.
Working with young people experiencing poor mental health across Bristol, who are in secondary mental health services and helping them and their mental health professionals in mapping and transitioning into a pathway to manage their mental health beyond their 18th birthday.
The post holder will ensure a cohesive, smooth transition for young adults from Children and Adolescent Mental Health Services (CAMHS) to Adult Mental Health Services (AMHS) that is evidenced as being significant in terms of their longer term recovery. They will also provide expert advice and support to all relevant stakeholders with regards to young adults who are receiving support from secondary mental health services who are approaching their 18th birthday.
NICE guidelines detail good practice in respect to the transition process and will help inform future practice.
