Job Overview

We have an exciting opportunity in Derby City for a substantive Band 7 District Nurse. The successful candidate will join our Lister House nursing team within PCCO PCN working alongside our other Band 7 District Nurse and Band 6 Senior Community Nurse.


The successful applicant would require community nursing experience, some knowledge of caseload management with a high standard of community nursing knowledge and clinical skills. You will be provided with training and support tailored to your needs to drive forward high quality care and teamwork, including a thorough induction programme, regular one-to-ones, peer supervision, working within an MDT and clinical training.


Responsibilities


  • Provide effective evidence based care to a defined population/Primary Care Network.

  • Delegate and manage caseload, monitor care standards, lead and manage performance of team members.

  • Assess, plan, implement and evaluate individualised patient care, utilising professional judgement at all times.

  • Provide clinical leadership and line management to less experienced nurses and other members of the multidisciplinary team in the delivery of high quality care.

  • Contribute to the continuous improvement of community nursing within your place, linked to the long‑term NHS plan.
