Responsibilities
  • Independently manage and prioritise a caseload across multiple Leeds community hubs.
  • Provide expert care and assessments for adults with varied needs, including frailty, falls, dementia, MSK, and neurological conditions (e.g., Parkinson’s, MS).
  • Apply the Mental Capacity Act when supporting clients in decision-making processes.
  • Recommend and provide basic equipment, aids, and adaptations to support independent living.
