• Full UK driving licence preferred to support community visits.

Responsibilities

• Deliver specialist occupational therapy assessments and interventions within the mental health team.
• Conduct comprehensive evaluations to support discharge planning from acute mental health wards.
• Work closely with MDT colleagues to develop safe and effective discharge plans.
• Provide tailored treatment strategies to promote independence, recovery and engagement in meaningful activities.
• Liaise with external agencies and community support services to ensure smooth patient transitions.
