About the Role
Successful applicants will work as part of a multidisciplinary team. You will hold a designated caseload and work generically as a care coordinator alongside your nursing functions. The Trust has regular nursing forums, with an emphasis on active involvement in continuing research and ongoing learning. We offer regular developmental learning sets, professional and line‑management supervision, yearly individual appraisals and a robust in‑service training/educational programme.
Key Responsibilities
- Flexible management of a defined caseload designed to meet the individual needs of clients and carers.
- Maintain a high standard of individualised client care, recognising factors and events that are likely to affect the mental, emotional and physical well‑being of clients/carers and provide advice and support when required.
- Act as a care coordinator by assessing clients’ needs and strengths through a collaborative approach, ensuring that all clients have care plans that are implemented and regularly reviewed/updated.
- Develop and maintain a high standard of practice, based on assessment, using a range of psychosocial approaches within a health and psycho‑social framework.
- Communicate/liaise effectively within relevant agencies to ensure that an integrated programme of care is provided throughout.
- Implement and/or participate in crisis intervention measures with the client/family or in conjunction with other agencies.
- Supervise and monitor the therapeutic effects and side‑effects of prescribed medication, providing the client with clear advice and information in accordance with Trust policy and the NMC standards for administration of drugs.
- Organise and participate in the admission, discharge and transfer of clients in accordance with Trust policies.
- Provide clients and carers with verbal and written up‑to‑date information on the services offered by the Trust, how to express their views, the complaints procedure and what they can expect from the service.
- Attend weekly clinical meetings to provide the team with relevant information in relation to caseload, referrals and to feedback on new assessment.
- Develop and maintain effective working relationships with in‑patient facilities regarding admission and discharge of clients.
- Facilitate carers’ assessments, provide the appropriate care plan to meet identified needs and monitor efficacy.
- Have knowledge and application of key legislation and policy such as the Mental Health Act, Care Programme Approach, Capacity Act and Care Act.
- Participate in the running of the clozapine and depot clinics.
Qualifications & Experience
- Band 6 Community Psychiatric Nurse position within community services in the borough of Merton.
- Experience working within a mental health setting post‑qualification, ideally a community mental health team.
- Ability to work autonomously, be flexible, energetic, and maintain a positive attitude.
- Excellent communication and interpersonal skills, with an ability to work collaboratively with individuals and across multidisciplinary teams.
- Committed to MDT working and partnership working, establishing good networks with our Recovery College and a variety of voluntary, statutory and independent agencies.
- Experience in developing personal recovery action plans and managing time‑limited interventions.
