Ealing Short Term Rehabilitation - Band 6 Community Physiotherapist

We have exciting opportunities for a Fixed term Band 6 physiotherapist to join our specialist multidisciplinary community rehabilitation service.
Our service has two bases. Interviews will be held in person at our office in Level 10 South, Ealing Hospital, London, or at the EDTC, Southall UB1 2SH.
Main Duties
The Ealing Short Term Rehabilitation (STR) team provides therapy‑led rehabilitation to help patients improve their abilities and achieve their goals, enabling people to regain independence and social integration effectively and efficiently after discharge from acute hospitals and community services in the borough. Therapists and rehabilitation assistants support patients for a period of up to six weeks, usually less if goals are achieved or care can be delivered in a different way. The team includes Physiotherapists, Occupational Therapists, and Rehabilitation Assistants, working mainly in patients' own homes.
Responsibilities
- Undertake physiotherapy assessment of patients with diverse needs through subjective questioning and objective assessment.
- Prioritise factors involved in the patient’s presenting condition through clinical reasoning based on current professional opinion, prognosis, and physical, social, emotional and mental health issues.
- Set goals for treatment with patients and/or their carers and ensure that care is patient‑centred and agreed by the patient and/or their carers.
- Maintain a high standard of clinical practice which is evidence‑based, using standardised assessment and outcome measures where appropriate.
- Independently screen and assess multi‑disciplinary referrals from a variety of sources, establishing priority, determining needs and initiating a plan of intervention.
- Manage a caseload of patients with a range of needs, utilising supervision and guidance from senior staff. Monitor, evaluate and review physiotherapy treatment within a multi‑disciplinary framework and modify as necessary, taking a key worker role where indicated.
- Ensure patient and service records are recorded and maintained on IT databases and that patient documentation is in accordance with trust and local policies.
- Prescribe, delegate and monitor the work of rehabilitation assistants and counter‑sign documentation.
- Actively be involved in service development, with support and guidance from senior therapy staff.
- Participate in departmental continuous professional development, education and training, audit and research projects.
- Maintain and update a Continuing Professional Development (CPD) portfolio.
Person Specification
Education / Qualifications (Essential)
- Degree in Physiotherapy
- State registration with HCPC
Experience (Essential)
- Previous experience of working in the UK or a similar health economy as a physiotherapist.
- Significant post‑registration working as band 5 or band 6 or equivalent in experience.
- Experience in a variety of core areas: older people/elderly rehab, medicine & general surgery, orthopaedics, falls, dementia, long‑term conditions.
- Previous experience of working with students or supervising students.
- Experience of completion of audit and service developments.
- Experience in assessing and treating patients with a variety of health conditions.
- Experience of working as an autonomous practitioner in a community setting.
- Previous general experience in community working, neurology, and pain management.
Skills and Knowledge (Essential)
- Excellent communication skills, fluency in written and verbal English.
- Demonstrates flexibility, problem‑solving, initiative and leadership skills.
- Able to demonstrate clinical and professional development.
- Ability to work independently and prioritise workload pressures with effective time‑management skills.
- Understanding of the principles of multidisciplinary teamwork.
- Understanding of the problems of disability and its effects, with a commitment to working with older people.
- Demonstration of knowledge of standards of practice, evidence and best‑practice guidelines.
- Able to demonstrate a good understanding of the physiotherapy role in rehabilitation and the roles of other professionals in the MDT.
- Ability to apply Health and Safety principles.
- Evidence of maintenance of a CPD portfolio with self‑directed learning and reflective practice.
- Knowledge of developments within NHS community rehabilitation and physiotherapy profession.
- Excellent IT skills including Outlook, Word, Excel, and relevant databases.
Desirable
- Membership and/or involvement in a national special interest group.
- Member of CSP.
- Experience of working in community neurology and pain management.
