Responsibilities
  • To admit patients and ensuring that all patients are assessed on the ward preoperatively.
  • To establish and maintain robust multi-disciplinary discharge planning arrangements in order to discharge patients safely and reduce length of stay, delayed transfers of care and re-admissions.
  • To proactively co-ordinate, review and case manage complex discharges and to facilitate simple discharges by engaging the multi-disciplinary ward team.
  • To ensure the discharge planning process is responsive to the needs of patients and their carers by means of joint working with the relevant acute, community and allied support staff.
  • Undertake assessment of patients\' to be admitted on the ward preoperatively and pre-procedure and assessing their discharge needs in appointed area of responsibility without supervision.
  • Co-ordinate and refer to appropriate therapy team if rehabilitation required.
  • Arrange and chair Multidisciplinary team meetings for individual patients with very complex discharge needs.
  • Refer and liaise with Social Service Department and other local agencies e.g., ICT as appropriate.
  • Work clinically on the ward and other areas where appropriate and as required, supporting the ward teams.
