Qualifications

  • Significant experience in medicines management, particularly within community or primary care settings.
  • Sound knowledge of drug usage, medicines optimisation, and stock control.
  • Experience in patient-facing roles, including medicines advice and counselling.

Responsibilities

  • Undertake home visits to support medicines management.
  • Investigate incidents involving missing controlled drugs.
  • Participate in team huddles and contribute to triage of discharge letters.
  • Identify and resolve medicines safety issues.
  • Collaborate with CCS teams and adult social care.
  • Maintain accurate records and adhere to policies and procedures.

Skills

Education

Current registration with HCPC
