Job description
Overview

An exciting opportunity to join a wonderful, dynamic and patient focused team providing high quality services to the local community. Whether it be supporting a team member with new skills or attending to a patient and advising their family, your role as a Community Nurse provides you with the opportunity to deliver a high standard of care to patients in their homes alongside developing your leadership skills supporting your team.

Responsibilities
  • Wound care, catheter care, palliative care, IV drug administration, medication/injection administration, diabetes management and many other diverse skills.
  • Travel around the local community; access to a vehicle and a full driver’s licence are required.
  • Shift patterns include 8am–4pm, 9am–5pm, 10am–6pm, 12pm–8pm; work 7 days per week with weekends, late shifts and bank holidays pro rata.
Qualifications

Registered Nurse with a full driver’s licence and access to a car. Ability to work the required shift patterns and to travel around the local community. Experienced in community nursing, supporting patients in home settings.

Benefits & Support

Preceptorship programme, specific clinical upskilling training and PNDs support. Two weeks of Trust induction and two weeks of local team induction with shadowing of DNs, TVNs, CNS and other clinicians.

Team & Collaboration
  • Work within an integrated team of District Nurses, Specialist Nurses, Community Matrons, Therapists and Health Care Assistants.
  • Collaborate with Local Authorities, Integrated Care Networks, GP’s and acute sector providers to facilitate care pathways that support early discharge and prevent hospital admission.
