Job description
Overview

We have an exciting opportunity in Intermediate Care in South Manchester. Pathway 2 includes rehabilitation beds at Buccleuch Lodge Intermediate Care Unit in West Didsbury and supporting patients in their home environment on our "Home Pathway".

About the Unit

Buccleuch Lodge provides inpatient care to 14 patients, offering rehabilitation with the aim of returning patients home with support. The unit has a unique community setting with dedicated nursing and therapy staff. The rehabilitation beds are accessible for both step‑down from a hospital setting and step‑up patients from home.

Responsibilities
  • Manage own caseload of patients, both in the bed‑based unit and in patients' own homes.
  • Carry out team and office‑based duties to ensure effective service delivery.
  • Work within the integrated service delivery model to avoid admissions, reduce length of stay, and improve patient experiences.
Qualifications and Requirements

Applicants must have a UK driving licence and access to a vehicle. Evidence of eligibility must be provided; failure to provide will prevent progression to interview.

The role requires a B3 Therapy Support Worker with compassion, forward‑thinking mindset, and strong teamwork skills. Experience in community rehabilitation is highly desirable.
