Job description

This is an exciting opportunity to join the East Locality in the role of Clinic healthcare support worker. The successful candidate will work as a Clinic HCA as part of the community nursing teams supporting non housebound patients that require clinical interventions that are deemed complex and therefore do not meet practice nurse criteria. You will need to be able to travel to the clinic sites. You will provide a holistic approach to individuals, that includes, following care plans and implementation of care., The post holder will work as part of the integrated Multidisciplinary team under the of the Clinical Team Manager. He / She will be accountable for the delivery of high quality care to patients within health centers. Working with other members of the IMT to provide care to and promote health/well being and independence for patients on the team caseload, working closely with other partners/stakeholders including GPs, Tissue Viability and vascular clinics.

The successful candidate should have experience of wound care, with a particular interest in caring for leg ulcers, compression therapy and lymphoedema care as well as catheter care.
