Job description

We are currently recruiting for two positions, with up to atotal of 60 hours per week available. We are happy to consider a full-timeequivalent (FTE) position and one part-time position, or alternatively, as twopart-time roles, depending on candidate availability and servicerequirements.

SAPA 5 is a Primary Care Network located in North Sheffield,made up of six practices within the S5 postcode area. We work in partnershipwith community services, mental health teams, social care, pharmacy, hospital, and the voluntary sector to deliver proactive, personalised,coordinated, and integrated health and social care for local residents closerto home.

The successful candidate will work closely with ourestablished clinical teams to deliver high-quality patient care in aninnovative and collaborative way. This role offers an excellent opportunity tofurther develop clinical skills within a positive, supportive, andexcellence-focused environment.

Main duties of the job

The post holder will be a qualified clinical pharmacist whoworks within their professional boundaries while supporting and collaboratingwith our general practice teams. In this role, they will receive guidance andmentorship from a designated clinical supervisor.

The post holder will offer primary support to generalpractice staff by managing prescription and medication-related queries. Theywill contribute to the repeat prescription process, handle acute prescriptionrequests, and carry out medicines reconciliation during transfers of care. Inaddition, they will help maintain safe prescribing systems and provide expertclinical medicines advice, ensuring the public and social care needs ofpatients within the GP practices are met.

Job responsibilities
  • Provide support to generalpractice staff by managing prescription and medication-related queries
  • Repeat prescription process
  • Acute prescriptionrequests
  • Medicines reconciliation during transfers of care
  • Maintain safe prescribing systems and provide expertclinical medicines advice, ensuring the public and social care needs ofpatients within the GP practices are met.
  • Patient facing clinical medication reviews, including structured medicationsreviews
  • Providing answers to medicine-related queriesfrom GPs and other healthcare teams
  • Management of common/self-limiting ailmentsthis includes signposting to community pharmacy or referring to GPs and otherhealthcare teams where appropriate.
  • Risk stratification using clinical reports,identify cohorts of patients at risk of harm from medicines.
  • Medicines quality improvement analyse,interpret and present medicines data to highlight issues and risks to supportdecision making.
Person Specification
Experience
  • GP practice experience is desirable, but not essential
  • Experience of using Emis Web.
  • Experience of using TPP SystmOne.
  • Minimum of 2 years experience as a pharmacist, demonstrated within a practice portfolio.
  • Have experience and an awareness of common acute and long-term conditions that are likely to be seen in general practice.
Qualifications
  • Completion of an undergraduate degree in pharmacy and registration with the General Pharmaceutical Council
  • Able to work flexibly across practices and within a multidisciplinary team
  • Must have own transport and full drivers license
  • Demonstrate understanding of the pharmacy role in governance and is able to implement this appropriately in the workplace.
