Qualifications

Responsibilities

  • Maintain high quality care management services by meeting participants' needs.
  • Perform care management functions including assessment and service plan development.
  • Complete Needs Assessment Tool and participant care plan.
  • Enter Care Management Activity for services provided.
  • Develop individual care plans with participants and their families.
  • Identify and mobilize resources to meet participants’ needs.
  • Facilitate participant choice of providers.
  • Arrange for needed services and entitlements.
  • Conduct home visits and monitor adequacy of care plans.
  • Ensure participant satisfaction and quality of services.
  • Participate in training and seek professional development.
