Role Overview

Aryll and Bute Health and Social Care Services are looking for highly skilled, motivated and enthusiastic individuals to join the Community Care Team Integrated Team at Cowal Community Hospital, Dunoon. The post holder will work within a multidisciplinary team, supporting patients/clients in their own homes with holistic assessment, care planning and delivery and help develop MDT working across primary, secondary and independent care sectors, with a key focus on frailty, prevention and early supported discharge.

Responsibilities
  • Provide high‑standard patient care to ensure the delivery of a high‑quality service to the local community.
  • Support the development of multidisciplinary team working across primary, secondary and independent care sectors, focusing on frailty, prevention and early supported discharge.
  • Deliver care within home and homely settings, using own initiative and flexibility.
  • Work full‑time on a rota basis, fixed‑term, 9am to 5pm over 5 days.
Qualifications and Requirements
  • Essential: valid driving licence and access to own vehicle for work purposes; pool car use may be considered on an ad‑hoc basis.
  • Experience working in community or similar settings is desirable.
  • Strong communication, interpersonal and organisational skills.
  • Ability to work independently and as part of a multidisciplinary team.
