A leading healthcare provider in Hemel Hempstead is seeking an Advanced Nurse Prescriber to manage ADHD treatments and provide holistic care to children aged 6-18. The role includes conducting assessments, liaising with multidisciplinary teams, and ensuring effective communication with families and healthcare providers. We are looking to recruit an Advanced Nurse prescriber who will work within the South West Herts Enhanced Primary Care Monitoringservice to provide support and intervention to children and young people withADHD (as well as co-morbidities) between the ages of 6 and 18.
The postholder will be confident in completing ADHD medication reviews utilisingspecialist nursing skills and using clinical interview techniques in order toascertain a childs response to medication and manage potential sideeffects. They will be expected to beconfident in ascertaining potential symptoms of co-morbidity such as ASD,mental health needs or physical health concerns and document informationaccurately and refer to appropriate local services.
You will beexperienced in titrating ADHD medication in children and young adults and confident with the formulary.
This is a clinic-basedrole, and you will carry out holistic assessments of patients referred asappropriate by HPFT into this primary care monitoring service. Clinics are based within the 4 localities of West Herts, Dacorum, St Albans, Hertsmere and Watford and 3 Rivers, this role will be based across 2-3 sites.
Applyknowledge of behavioural management strategies and evaluate responses to theseand medication.
Ability to communicate orally, electronically and in writing withchildren, young people, and families.
Correspond with GP supervisors in the team, multi professional agencies, families, children services and voluntary/ statutory services. Attend weekly MDT meetings with senior teamto discuss and review caseload regularly. Attend monthly MDTs to discuss cases with CAMHS ADHD service.
Knowledge of referral pathway for complex cases.

Job Responsibilities
Work autonomously in clinic to provide holistic reviews to children and young adults in relation to the management of their ADHD medications and to adjust drug treatment appropriately with the support ofsenior team and working within agreed protocols and guidelines with therecommendation of continued prescribing shared to the child/ young persons GPwithin recommended guidelines of the NMC medicine management code of conductand NICE guidance.
Provide anintegrated service working closely with wider team and liaising with colleaguesin HPFT and other services as required.
As anautonomous practitioner, be responsible for monitoring patients with an ADHDdiagnosis on medication through nurse-led clinics, communicating change toother members of the team and giving guidance on pharmaceutical treatment ofChildren and Young People up to the age of 18. The post-holder will be able totake a holistic approach to care, considering a variety of aspects which mayimpact on the childs function and behaviour and ensuring to hear the child'svoice within their review process.
To attendvarious multi-agency meetings, write reports and plan care, acting as thepatient advocate when appropriate and to work in accordance with safeguarding childrenspolicies. Raise and discuss complexcases with supervising GP and ensure appropriate onward referral made.
Contribute tothe development of leaflets and information regarding medication and ADHD forchildren, families, and professionals.
Ensure theeffective and efficient use of physical and financial resources.
Collect,collate, evaluate, and report information and maintaining accurate patientrecords.
Ensure theservice is monitored on a regular basis and data is available as a record toprovide statistical information to Clinical Leads and others and to providedata to relevant local and national bodies to audit service as appropriate.
