Advanced Nurse Practitioner/Paramedic Practitioner- Prescribing

Avisford Medical Group are looking for an experienced Prescribing advanced nurse practitioner or Paramedic Practitioner to join our friendly team.

We are a friendly, forward-thinking and highly supportive training Practice in West Sussex. We have a strong and stable partnership and have a dedicated team of highly skilled clinicians and non clinical members of staff.

The advertised role is a new role, and we are looking for someone motivated and keen to join our ever growing workforce.

Main duties of the job

TheAdvanced Clinical Practitioner(ACP) has been assessed as competent inpractice using their expert clinical knowledge and skills. They have thefreedom and authority to act and make autonomous decisions in the assessment,diagnosis and treatment of patients.

Thepost holder works at the level of advanced clinical practice and has developedand can evidence the underpinning competencies applicable to the specialty orsubject area, i.e., the knowledge, skills and behaviours appropriate to the healthcareprofessionals (HCP) setting and job role.

Therole provides an opportunity for patients to avoid the need to bereferred to a GP, thereby enabling them to receive timely care and negateunnecessary delay in receiving treatment.

An ACP is autonomous in making decisions basedon assessment, diagnosis and interpretation of test results and will initiate,evaluate and modify a range of interventions which mayinclude prescribing medicines, therapies, lifestyle advice and care.

Job responsibilities

Thefollowing are the core responsibilities of the Advanced Clinical Practitioner.

  • a. Practise in compliance with their respectivecode of professional conduct and within their scope of practice, beingresponsible and accountable for their decisions, actions and omissions at thislevel of practice
  • b. Demonstrate a critical understanding of theirbroadened level of responsibility and autonomy and the limits of own competenceand professional scope of practice, including when working with complexity,risk, uncertainty and incomplete information
  • c. Act on professional judgement about when toseek help, demonstrating critical reflectionon own practice, self-awareness, emotional intelligence, and openness to change
  • d. Work in partnership with individuals, families andcarers, using a range of assessment methods as appropriate
  • e. Demonstrate effective communication skills, supportingpeople in making decisions, planning care or seeking to make a positive change
  • f. Use expertise and decision-making skills toinform clinical reasoning
  • g. Initiate, evaluate and modify a range of interventions which may includeprescribing medicines, therapies, lifestyle advice and care

  • i. Managepatients presenting with a range of acute and chronic medical conditions,providing subject matter expert advice
  • j. Implementand evaluate individual specialised treatment plans for chronic diseasepatients
  • k. Identify,manage and support patients at risk of developing long-term conditions,preventing adverse effects to the patients health

  • m. Actively engage in peer review to inform own and otherspractice, formulating and implementing strategies to act on learning and makeimprovements
  • n. Actively seek feedback and involvement from individuals,families, carers, communities and colleagues in the co-production of serviceimprovements
  • o. Critically assess and address own learning needs,negotiating a personal development plan that reflects the breadth of ongoingprofessional development across the four pillars of advanced clinical practice(clinical practice, leadership and management, education and research).
  • p. Providechronic disease clinics, delivering patient care as necessary, referring patientsto secondary/specialist care as required
  • q. Maintainaccurate clinical records in conjunction with extant legislationand to ensure SNOMED CT codes are used effectively
  • r. Develop,implement and embed an effective call/recall system
  • s. Critically appraise and synthesise the outcome ofrelevant research, evaluation and audit, using the results to underpin ownpractice and to inform that of others
  • t. Develop andimplement robust governance systems andcontribute to practice targets (QOF, etc.), complying with local and regionalguidance
  • u. Chaperonepatients where necessary
  • v. Supportthe team in dealing with clinical emergencies
  • w. Supportthe team with all safeguarding matters in accordance with local and nationalpolicies
  • x. Understandpractice and local policies for substance abuse and addictive behaviour,referring patients appropriately
  • Deliver opportunistic health promotion whereappropriate
Person Specification
Experience
  • Experience of practice within the four pillars.
  • Job plan that demonstrates advanced clinical practice and has equity with peers working at this level.
  • Experience of prescribing and undertaking medication reviews.
  • Experience of working in a primary care environment
Other Requirements
  • Enhanced Disclosure Barring Service (DBS) check.
  • Occupational health clearance.
  • Meet the requirements and produce evidence for revalidation.
  • Evidence of continuing professional development (CPD) commensurate with the role of an ACP.
  • Access to own transport and ability to travel across locality on a regular basis.
  • Flexibility to work outside core office hours.
Qualifications
  • Educated to Masters degree as detailed within the HEE document tilted Multi-professional framework for advanced clinical practice in England .
  • Qualified independent prescriber - V300.
  • Registered on their appropriate professional register.
  • Teaching qualification.
  • ALS and PALS
Clinical Knowledge and Skills
  • Understand the importance of evidence-based practice.
  • Broad knowledge of clinical governance.
  • Ability to record accurate clinical notes.
  • Ability to work within own scope of practice and understand when to refer to GPs.
