Job description

Vine Medical Group is recruiting for a full or part time Advanced Nurse Practitioner to join our Same Day Care Team.

Our new team member will work both independently and withina multi-disciplinary team environment, to provide safe, holistic care to ourpatients. This will include remote and face to face appointments, reactive andproactive home care visits. You will berequired to work within your professional competence and boundaries to provideboth urgent and planned care to patients.You will need to prioritise and triage the needs of patients, making anynecessary referrals for investigations or to secondary care in the appropriatemanner.

This role is particularlysuited to an autonomous clinician with a proven track record of delivering highquality care within the Primary Care setting with experience in dealing with awide range of minor ailments.

Main duties of the job

Job responsibilities
  • To provide a first point of contact within the service forpatients presenting with undifferentiated physical and mental healthconditions, offering holistic assessment. To include history taking, physicalassessment and treating minor illness and ailments, undertaking investigationsto inform clinical decision-making to establish a diagnosis and management planin collaboration with the patient and their families or carers.
  • To request diagnostic investigations and interpret within agreed protocols e.g.x-ray, blood tests, scans, etc
  • Work within clinical boundaries, demonstrating safe and competent clinicaldecision-making, delivering high quality clinical care and maintaining patientconfidentiality at all times
  • Provide telephone or video consultations, including the safe and effectivetriage of acutely unwell patients
  • Conduct reactive home visits to those acutely unwell patients, assess, plan,treat and arrange admission to hospital if required, for the practice andwithin the PCN
  • Future intention to conduct proactive home visits to housebound patients withlong term conditions, including but not exclusively dementia, diabetes,respiratory disease.
  • Make appropriate referrals to primary, secondary and social care services,following practice (and/or locally agreed) pathways and protocols.

Whereappropriately qualified:Supply,administer and prescribe medicines as indicated (and within competencies) toaddress patient need, in accordance with medicines management policy, patientgroup or patient specific directives or NMC standards

  • Promote self-care and educate service users to allow them to be empowered tomake informed choices about treatment
  • To appropriately request and collect pathological & microbiology specimensincluding venepuncture, swabs, urine culture in line with local guidelines andprocess results as required actioning them in a timely manner in line withprotocols
  • Identify, manage, and support patients at risk of developing long-termconditions, preventing adverse effects on the patients health, promotepatients to live healthily and encourage principles of self-care
  • Recognise and manage anaphylaxis, according to the current UK ResuscitationCouncil guidelines
  • Perform Cardio-pulmonary Resuscitation and the correct use of Automated Defibrillatorfollowing training, according to the current UK Resuscitation Councilguidelines
  • To provide general health promotion education, advice on national screeningprogrammes and guide/refer patients to appropriate services or other healthprofessionals as deemed appropriate
  • Provide information and advice on prescribed or over-the-counter medication, onmedication regimens, side effects and interactions as appropriate and withinyour level of competency
  • Support the clinical team with all safeguarding matters, in accordance withlocal and national policies
  • Accurate recording of care provision onto patient records ensuring correctcoding of all procedures and information using approved methods andtemplates
  • Chaperoning and assisting patients where appropriate who are being examined byanother clinician
  • Conduct routine and post-use inspections of emergency equipment and medications
  • Clearing and re-stocking own clinical room/trolley at end of the sessionensuring stock rotation and date checking
  • Cleaning and maintaining clinical equipment, reporting faults promptly
  • Preparing and maintaining environments and equipment before, during and afterpatient care interventions
  • Actively participate in all aspects of infection prevention and control andchampion this within the practice
  • Assisting with the collection and collation of data on needs related to healthand well-being and support QOF requirements and data completion
  • Actively participate in seasonal and special projects as requested e.g. flucampaign
  • Be familiar with patient safety policy and reporting (QUASAR) and awareness ofthe process and key stakeholders for safeguarding referrals
  • Maintain accurate patient / clinical records in conjunction with Practicepolicies and current legislation
  • Ensure adherence to confidentiality, data protection, and informationgovernance requirements
  • Work safely at all times in accordance with legislative requirements andPractice Policy and Procedures
Person Specification
Qualifications
  • Registered Nurse Practitioner / Advanced Nurse Practitioner with NMC
  • Qualified to Master's degree or equivalent
  • Full UK Driving Licence
  • Physical Assessment and Minor Illness Training
  • Accredited training in any of the following: Urgent care, Chronic disease management or frailty
  • Non-Medical Prescriber

  • Clinical examination skills across a diverse range of patients including both children and adults
  • Able to work autonomously within your scope of practice whilst also having an understanding of when to access the wider clinical team
  • Able to build positive rapport with a diverse range of patients and colleagues
  • Good organisational and time management skills with the ability to adapt to changing priorities and tasks
  • Ability to work as a team member and autonomously
  • Effective communicator, able to communicate complex and sensitive information to patients and carers, recognising the need for alternative methods of communication to overcome differing levels of understanding, cultural background and preferred ways of communicating
  • Confident IT user with accurate data entry skills and a working knowledge of Windows and Office 365
  • Working knowledge of EMIS and Docman medical systems
Experience
  • 3 years+ experience of working as an Nurse Practitioner / Advanced Nurse Practitioner
  • Background of working in a fast-paced Primary Health Care or acute environment is essential
  • Proven record of acting in roles where you have acted autonomously and responsibly for the assessment, treatment and/or appropriately referral patients to specialist services when required
