Are you an experienced Advanced Nurse Practitioner looking to make a difference in primary care? Service Care Solutions is seeking a dedicated Advanced Nurse Practitioner to join a Primary Care Home Visiting Service based in Norfolk. This is an exciting locum contract opportunity starting in January 2026, offering flexible hours and the chance to work within a multidisciplinary team committed to delivering high-quality patient care.

Job Overview: The Advanced Nurse Practitioner

As an Advanced Nurse Practitioner, you will be responsible for delivering evidence-based services, working autonomously and supporting chronic disease management, health promotion, and disease prevention initiatives. You will operate within your scope of practice, prescribing medications as appropriate and developing personalized care plans for various patient populations. Your role involves supporting the clinical team through policy review, developing health programs, and identifying patients at risk of long-term conditions.

Candidate Responsibilities
  • Deliver evidence-based clinical services within scope of practice
  • Prescribe medications according to PGD and review their effectiveness
  • Develop, implement, and evaluate individual treatment plans for acute and non-acute conditions
  • Support health promotion and wellbeing programs
  • Manage patients at risk of long-term conditions to prevent adverse health outcomes
  • Work collaboratively within a multidisciplinary team, providing clinical supervision where needed
  • Lead reviews of clinical policies and procedures
  • Provide routine nursing care following NICE and NSF guidelines
  • Support the development of individual care plans for chronic disease management
