Advanced Clinical Practitioner - Paramedic - Experienced Nurse

ThePCN is seeking an experienced Advanced Clinical Practitioner, Paramedic, or ExperiencedNurse to support member practices by delivering high-quality clinical care tohousebound patients and care/nursing home residents.

The rolefocuses on the assessment and management of acutely unwell patients incommunity settings, supporting same-day demand, improving continuity of care,and reducing avoidablehospital admissions.

Part time and Full time roles will be considered

Main duties of the job
  • Provide same-day and urgent clinical assessments for housebound patients
  • Undertake comprehensive assessment, diagnosis, and management of acute and chronic conditions within scope of practice
  • Independently manage a clinical caseload, escalating to GPs or secondary care when required
  • Prescribe and manage medications

Blackpool South PCN is made up of four GP Practices with a weighted list size of 37,500 spread across a fairly compact urban area.

The PCN formed in 2019 with no changes to its original members to date.

Job responsibilities

Care Home & Housebound Services

  • Deliver proactive and reactive care to residents in nursing and residential homes
  • Support structured care home rounds and urgent reviews
  • Contribute to anticipatory care planning, frailty management, and end-of-life care
  • Work closely with care home staff to provide clinical advice and guidance

Multidisciplinary Working

  • Work collaboratively with GPs, Practice staff, ARRS roles, community nursing teams, and social care
  • Participate in MDT meetings and care planning discussions
  • Maintain effective communication with patients, carers, and families

Governance & Quality

  • Maintain accurate clinical records using GP clinical systems
  • Adhere to PCN and practice policies, clinical governance, and safeguarding procedures
  • Contribute to service development and quality improvement initiatives across the PCN
Person Specification
Qualifications
  • Registered healthcare professional NMC or HCPC
  • Qualified ACP, Paramedic, or Registered Nurse
  • Independent prescribing qualification V300 or equivalent
  • Demonstrable experience in assessing and managing acutely unwell patients
