A community health provider in Tees Valley is seeking an Advanced Clinical Practitioner for HMP Holme House. Spectrum have an exciting opportunity to join our team at HMP Holme House working as an Advanced Clinical Practitioner supporting our primary care team.
Advanced clinical practice is delivered by experienced, registered health and care practitioners. It is a level of practice characterised by a high degree of autonomy and complex decision making. This is underpinned by a master’s level award that encompasses the four pillars of clinical practice, leadership and management, education and research, with demonstration of core capabilities and area specific clinical competence using the multi-professional advanced clinical practice framework.
The Advanced Clinical Practitioner (ACP) will have the ability to manage clinical care in partnership with individuals, families and carers, where appropriate. This will include the analysis and synthesis of complex problems across a range of settings, enabling innovative solutions to enhance people’s experience and improve outcomes. ACPs are highly experienced and educated members of the care team who are able to diagnose and treat conditions and refer onwards to appropriate specialists if needed.
