A healthcare facility in Swindon is seeking an experienced Advanced Clinical Practitioner to join its primary care team. In this role, you will assess, diagnose, and manage long-term conditions for residents in care homes and housebound patients. Youll assess, diagnose, prescribe, and managefrailty and long-term conditions, working closely with GPs, pharmacists,nurses, and care staff to improve outcomes and reduce hospital admissions.

You will be working within the Enhanced Health in Care Homes(EHCH) model supporting people living in care homes who can expect to receivethe same level of support as if they were living in their own home.
Supporting our housebound patients to improve the managementof LTCs and help reduce admissions.
Triaging and attending acute home visits in rotation withthe clinical team.
Our aim is working together to provide high quality,accessible, patient focused and sustainable healthcare.
Working with our existing clinicians to support the work ofthe GPs in the practice with care-home visits and ward-rounds, houseboundpatients and acute home visiting.
Weekly care home rounds, liaising closely with care homestaff, practice GPs and colleagues in support of their residents.
Occasionally support the GPs by triaging and attending acutehome visits in rotation with the established clinical team. To manage acuteillnesses in patients who are not able to attend the surgery, may includearranging admissions to hospital or liaising with community teams, palliativecare teams and social care.
Delivering support to our housebound patients. To improvemanagement of LTCs, help with decisions around ceilings of treatment andreduce hospital admissions.
To deliver a high standard of patient care, using advancedautonomous clinical skills, and a broad in-depth theoretical knowledge base.
To contribute to the QOF/IIF targets for the Practices andsupport the ongoing development of both the care home and visiting service.
The Merchiston practice was established around 1860 in the village of Stratton-St-Margaret. Until the 1950s, Stratton was a small farming community and it still retains its separate identity, despite now being a suburb of Swindon. The Partnership moved from the original Doctors house into new purpose-built premises in 1992.
Merchiston Surgery is the larger Practice with a raw patient list of 13,500 (13,700 weighted list)
We are a teaching practice with 2 Education Supervisors, 2 Clinical Supervisor, supporting 3 Trainee GPs. Rated excellent by the Swindon GP QA Panel for our trainee placement.
Following a recent inspection we are rated Good across all domains by the CQC.

Responsibilities
Clinical practice:
To include ward rounds at nursing/care homes to deal withclinical queries some which may be urgent to avoid admission, medicationreviews, referrals and end of life care.
Manage requests from patients requiring acutehome visits. Triaging appointments as well as visiting.
Support in the care of housebound patients.
Ensure the highest standards of treatment and care for allpatients (including complex patients, both acute and chronic). This willinvolve diagnosis, implementation of care plans to ensure physiological andpsychological factors are taken into consideration and ongoing evaluation ofany plans to ensure patient compliance.
Working closely with our team of Pharmacists to prescribeand review medication for therapeutic effectiveness, appropriate to patientneeds and in accordance with evidence-based practice and national and practiceprotocols, and within scope of practice.
Health promotion and motivation of patients to improvegeneral health.
Communicate effectively with patients and carers/care homestaff (nurses/carers), recognising the need for alternative methods ofcommunication to overcome different levels of understanding, culturalbackground, and preferred ways of communicating.
Recognising issues and gathering sufficient information torefer (e.g. drugs; domestic violence; child protection; vulnerable adults;senior patients; social problems).
Manage and assess risk within the areas of responsibility,ensuring adequate measures are in place to protect staff and patients.
Be mindful of work areas and practices to ensure they aresafe and free from hazards and conform to health, safety and securitylegislation, policies, procedures and guidelines.
Follow all necessary infection-control measures according tolocal and national guidelines.
