Advanced Clinical Practitioner (ACP) in General Practice

Brewood Medical Practice have an exciting opportunity for a motivated, resilient, experienced Advanced Clinical Practitioner to join our friendly team.

We are a semi-rural, part-dispensing General Practice with a list size of approx. 10,700 patients

Main duties of the job

We are looking for a experienced Advanced Clinical Practitioner to perform as an autonomous practitioner working independently and with other health care professionals to assess, diagnose and treat patients with a range of acute, non-acute and chronic medical conditions, initiating direct referrals as appropriate.

To manage daily surgeries comprising offace-to-face consultations, telephone consultations, house visits and telephonetriage work.

Undertake clinical practice at an advance level using expertknowledge and clinical skills to deliver holistic care to people accessingprimary care services.

Undertake home visits for chronic and acutely unwellpatients for certain medical emergencies.Prescribe medication as necessary for areas of chronic disease.
