A healthcare organization in Southampton is seeking an Advanced Care Practitioner to deliver proactive home visits and acute care for housebound patients and care home residents. The role involves comprehensive assessments, treatment plans, and close collaboration with multidisciplinary teams. A relevant advanced nursing qualification and registration are essential. An annual salary range of £58,000 to £61,000 based on experience is offered.
We are looking for an Advanced CarePractitioner (Advanced Nurse Practitioner / Paramedic), to join our already establishedHome Visiting and Care Home team. The practitioner will provide on the dayacute visits for housebound patients and care home residents within the PCN, aswell as delivering proactive home visits to support the ongoing health needs ofour population.
The post holder will be required to provide on the day visitsfor housebound patients who are acutely unwell and need a clinical assessment,as well as pre-planned proactive care assessments. The practitioner will alsoneed to visit patients living in care homes for both acute and proactive care,this will be in the form of either a ward round or an acute visit. The clinicianwill be responsible for prioritising visits according to clinical need and willbe required to take a comprehensive history and devise a treatment plan. Thiscould include liaison with the GP, secondary care and community colleagues tosupport coordination of patient care.
As part of the proactive care service delivery, the postholder will be required to complete comprehensive frailty assessments, advancedcare plans, end of life care planning, and medication optimisation whileworking closely with a wider MDT to identify patients who require input fromother services or agencies.

Directly admitspatients to secondary care hospital in acute medical need
Refers patients, asappropriate, to other members of the multidisciplinary team, secondary care andto other statutory and voluntary organisations
Interprets a range ofdiagnostic tests
Works withinprofessional guidelines and codes of conduct
Works within allrelevant policies and procedural guidelines e.g. infection control,chaperoning, risk management
Communicates informationwell and sensitively to patients and carers having due regard toconfidentiality
Contributes towardstargets under the primary Care Network Enhanced service and the Quality andOutcomes Framework
Undertake and completesquality improvement projects where necessary
Undertake annualreviews and care plans for patients including medication reviews (with supportof pharmacists where necessary).

Helps to deliver vaccinationprogrammes for our housebound populations and care home residents

Participation in an annualindividual performance review, including taking responsibility for maintaininga record of own personal and/or professional development
Taking responsibility for owndevelopment, learning and performance and demonstrating skills and activitiesto others who are undertaking similar work.
