Overview

Sanctuary Personnel is seeking a Social Worker to join the Hospital Discharge Team in Scunthorpe on a full-time locum contract.

Location: Scunthorpe, UK

Contract: 3 months, full-time

Salary: £35 per hour

This role involves supporting patient transitions from hospital to home, ensuring care plans align with patients’ needs and wishes within a fast-paced hospital environment.

What you will do
  • Participate in home visits and complete Care Act Assessments to determine community care needs.
  • Efficiently liaise with Scunthorpe General Hospital's integrated discharge team to facilitate seamless patient discharges.
  • Conduct Mental Capacity Assessments and Best Interest Decisions where necessary to protect patient welfare.
  • Manage a caseload with varying complexity, providing tailored interventions that promote independence and well-being.
  • Collaborate with multi-disciplinary teams to identify innovative solutions for patient support and discharge.
  • Maintain comprehensive records following all regulatory requirements and best practices.
