Role Overview

about the Role and our Ideal Candidate
Join Adult Social Care as a Social Worker and receive a Welcome Bonus* of £1000 paid in your first month's salary or a Retention Bonus* of £1000 paid for every complete year in service (* subject to payscale).

Key Responsibilities
  • Undertaking Care Act 2014 Assessments – identify individuals’ care and support needs, ensuring a personalised approach that emphasises the best outcomes
  • Completing Care Act 2014 Reviews – evaluating the strength and suitability of an individual's care and support plan, ensuring that it continues to meet their needs and preferences
  • Managing Safeguarding enquiries – investigating concerns about potential abuse or neglect, ensuring the safety and well-being of vulnerable individuals
  • Mental Capacity assessments – determine an individual's ability to make specific decisions regarding their care and treatment
  • Undertaking duty on rota basis
  • Undertaking DSTS and CHC
  • Court related work
Candidate Profile
  • To be registered with Social Work England and hold a degree in Social Care or similar
  • At least 2 years post ASYE working experience as a UK registered social worker
  • Have experience of working in a multidisciplinary team setting
  • Experience in undertaking complex Mental health and Physical health cases
  • Have sound knowledge and ability to deliver social work support to service users and their families
  • Ability to communicate complex social issues clearly to both patients and professionals
  • Ability to plan and coordinate social care plans and aspects of patient discharge
  • Please note: Social Workers may be required to travel outside the Borough to visit clients as part of their caseload. Further details regarding the caseload and travel expectations will be provided during the interview process.
