Adult’s Social Worker, Integrated Network Co – Coordination Service, North London

Pay rate: £32.52 per hour
Contract role, Integrated Network Co – Coordination Service

Pertemps are recruiting for experienced Adult’s Social Worker in Islington.

Responsibilities
  • As Social Worker with the Integrated Network Co – Coordination service (INC), you will be a member of a multi‑disciplinary team of GP, Mental Health, DNs, Age UK and Housing.
  • You will attend 3 to 4 network meetings per week on line to represent LBI. You would be expected to jointly discuss people with complex physical / mental health and/or social care situations with partner agencies.
  • You will be expected to contribute a social care perspective to case discussions, articulating social care legislation, approaches and values and explaining the remit of adult care services.
  • Act as a gateway to Adult Social Care and housing, actively taking referrals back to the appropriate teams and liaising with professionals as needed. You will need to actively build relationships and develop links with the other professionals and organisations involved in the networks. You will need to ensure network discussions are recorded in LBI case notes and providing feedback to named professionals in a timely way.
  • Manage the INC in-tray on LAS and liaise directly with the INC admin support team to manage referrals from social care, housing and third sector partners. You will act as the liaison with relevant teams within social care, housing and the third sector to support with case identification to ensure those cases that could benefit from a multidisciplinary approach are brought to the network meetings for discussion.
  • There is the opportunity to participate in other activities, which link into the further development of the INC service and the Community Services working agenda in the borough more widely.
