We're looking for a talented health or social care professional to join our leadership team in Welwyn Garden City Hertfordshire as Service Manager. The role will provide a genuine opportunity for you to develop your management skills and make a difference.

About the Role

You will work closely with the Service Line Lead and management teams to ensure the effective co-ordination and operational delivery of community health & social care services in HPFT.

Key Responsibilities
  • Assist the Service Line Lead to deliver a service that aligns with the Trust’s transformation agenda and implementation of care pathways, ensuring a cohesive, effective and seamless service for users.
  • Ensure the effective delivery of community based care services using a whole systems approach to achieve required performance levels.
  • Support the management of relationships with primary care general practitioners, HCS, PCT and 3rd sector organisations as required.
  • Responsible for the provision of quality care within a complex range of health and social care services that interface with HPFT and other providers.
  • Accountable for the effective operational management of all care pathways, requiring effective liaison and co-ordination with secondary care clinicians and managers.
  • Manage within established frameworks and processes to monitor capacity on a day‑to‑day basis, ensuring safe escalation arrangements and full safety with the ability to respond where team capacity issues present.
  • Ensure compliance with all statutory requirements e.g. CQC, HSE and implement relevant assurance systems to demonstrate compliance for the service.
Person Specification
Qualifications
  1. First or higher degree.
  2. Professional qualification in relevant area of health or social care.
  3. Postgraduate qualification or formal management training.
Experience
  1. Demonstrable success at managing change and development.
  2. Demonstrable success in quality improvement.
  3. Significant management experience in relevant area of health or social care.
  4. Demonstrable success in performance management and appraisal with multi‑disciplinary staff group.
Communication Skills
  1. Ability to organise and prioritise a complex workload.
  2. Ability to produce clear, written statistical and financial reports.
  3. Ability to communicate effectively at all levels of involvement including Service Users and Carers.
  4. Ability to influence and motivate staff to engage with and adopt change and deliver challenging targets.
Analytical Skills
  1. Ability to analyse critically financial performance and clinical data sets and present conclusions.
  2. Innovative thinker with ability to recognise underlying causes of and overcome barriers to change.
