Job description

We are looking for an Acute Response Practitioner qualified band 7 Registered Health / Allied Health Professional who has successfully completed PACR module or equivalent.

Community Crisis Response Team (CCRT) requires skilled and experienced practitioners tojoin us to support our community responses and the development anddelivery of Plymouths virtual ward programme.

This in an exiting time to join the team as our UrgentCommunity Response agenda expands and the virtual ward initiative develops tohelp support people in crisis to remain in their homes. CCRT is amulti-disciplinary team responding to people who need support to preventunnecessary hospital admissions. We are keen to hear from you if you are anurse, OT, Physiotherapist, Paramedic and have the skills, knowledge and experience to work as an Acute Response Practitioner.

As a urgent community response team CCRT work towards providing NHS England expectations, within this scope providing a 7 day service, operational hours 8am until 8pm.

This role maybe eligible for sponsorship under the Skilled Worker route; please refer to the Direct Gov website for more information with regards to eligibility.

Main duties of the job

To providereactive and proactive assessment, treatment and onward referral for people and caseload management whoare frail or have complex needs. This will be provided as part of communityacute response service in patients homes or in their usual residence.

To preventhospital admission and patient deterioration for people with an acute episodeor exacerbation of a long term condition.

This is acommunity post, working closely with primary care and will be supporting virtual wards.

To providehigh quality person centered care which always considers peoples safety,privacy and dignity using advanced clinical knowledge that is evidence based.

To undertakeindependent assessment and management of service users to incorporatepharmacological considerations, where appropriate making/supporting complex clinicaldecisions regarding service user management and clinical outcomes.

To provideadvanced clinical advice, leadership and support ensuring the needs of theservice are met by exercising and demonstrating high levels of clinicaljudgement, critical analysis and advanced decision making skills.

To managechange, to monitor and improve standards through supervision, evidence basedpractice, clinical audit, research and education. To promote and demonstratebest practice by integrating evidence into practice.

Experience

• Evidence of recent professional development in an up-to-date portfolio.
• Non-Medical Prescriber or competent in the administration and use of emergency care PGDs.

• Experience at Registered Practitioner Band 6 level or equivalent.

Qualifications

• Registered practitioner to degree/diploma level supplemented by minimum PG dip (120 credits) plus MA/MSc or working towards in a relevant subject or demonstrable extensive experience in the relevant specialty.
• Membership of the relevant Professional Body.
• Learning and Assessing in Practice Qualification or equivalent practice assessors training.
