Fella & Delilah is the direct-to-patient clinic for busy families to get to a healthier weight.

Fella is optimized for busy bigger guys who want a no-nonsense solution to getting healthier. Delilah is built for strong women in their prime who want a sustainable solution to feel their best.

We use affordable, effective weight loss medication to solve our customers’ most pressing healthcare need, then aim to remain their core healthcare provider for the rest of their life — expanding to hormonal health, mental health, longevity, and primary care.

GLP-1 medications are a f*ing big deal. We’re in a remarkable & rapid period of change which will radically reshape the patient & cash flows in the healthcare system.

Our mission is to accelerate the global take-up of GLP-1 medications among busy families to help solve obesity worldwide. From this, we’ll become the world’s leading consumer healthcare brand.

On being a Fractional Medical Director at Fella & Delilah

As our first Medical Director in Canada, you’ll help define what safe, scalable, and compliant care looks like for Canadian Fellas. You’ll serve as the responsible physician providing oversight for our clinical model and medical providers, helping us deliver high-quality async care while navigating Canada’s regulatory frameworks.

This is a part-time, remote contractor role (5-10hrs/month to start) with room to grow as our Canadian operations expand.

This isn’t just a clinical role — it’s a chance to shape Fella’s Canadian care model from the ground up. We’ve scaled rapidly in the U.S., and Canada is our next major market. You’ll be our Fractional Medical director, and you’ll help define what great care looks like here.

You’ll work closely with our future Nurse Practitioner, Clinical Ops Lead, and founder. You’ll also help us shape protocols, workflows, and our overall clinical experience in Canada.

Key Responsibilities

Right away:

  • Serve as the Responsible Physician for reviewing and signing off on Fella’s protocols, patient safety practices, and medical SOPs in Canada.

  • Review and approve clinical protocols for GLP-1 prescribing and other medical care in alignment with provincial guidelines and College standards.

  • Provide oversight and escalation support for complex clinical cases or edge scenarios, including adverse events and prescription eligibility edge cases.

  • Work cross-functionally with the Ops, Product, and Regulatory teams to ensure compliance with medical legislation and licensing frameworks in each province.

  • Build and document defensible medical justifications for our asynchronous and low-touch care model (e.g. rationale for async, patient triage logic, low visit frequency).

  • Help us navigate interprovincial medical leadership rules, including supervision requirements and medical delegation if needed.

Over time (as Canada scales):

  • Oversee or delegate oversight of additional NPs and physicians as we scale province by province.

  • Audit and continuously refine medical decision-making in our workflows (e.g. prescribing logic, escalation pathways, lab result thresholds).

  • Advise on local prescribing policies, controlled substance management, and pharmacy relationships as our care offering expands (e.g. longevity, testosterone, or future adjunct meds).

Need to Have
  • Licensed MD or DO, active, unrestricted license in at least 3 Canadian provinces (BC, ON, AB)

  • Eligible to serve as Medical Director / Responsible Physician per College regulations.

  • Comfortable providing oversight for asynchronous and low-touch care models, especially in primary care or obesity medicine.

  • Experience with GLP-1s, TRT, or other chronic condition prescribing.

  • Willingness to create protocols, respond to edge cases, and help document our medical rationale for compliance.

  • Strong understanding of clinical ethics, medical liability, and regulatory frameworks in Canada.

  • Strong documentation and process-thinking skills — you like building systems and protocols, not just practicing medicine.

Nice to Have
  • Experience as a Medical Director or supervising physician in a virtual care or startup setting.

  • Familiarity with obesity medicine, endocrinology, or men’s health.

  • Interest in longevity medicine or familiarity with treatments like metformin, NAD+, or peptides.

  • Experience interacting with College regulatory bodies, insurance carriers, or pharmacists.
