Qualifications

• Experience with mental health and addiction challenges.
• Strong understanding of community resources and healthcare systems.
• Brain Story Certification considered an asset.

Responsibilities

• Conduct comprehensive health assessments.

• Facilitate referrals and liaise with healthcare providers.
• Maintain accurate documentation and contribute to program evaluation.
