Overview

The RN Central Access and Referral Coordinator, under the general direction of the Community Team Manager, will work as part of an inter-professional regional Seniors Mental Health Behaviour Support Services team. The role functions as the entry point for Community Seniors Mental Health Behaviour Support Services, providing initial screening and triage assessment for clients living with complex cognitive mental health needs. The Central Access and Referral Coordinator collaborates with SMH BSS team members, clients and their care partners, other care providers and community partners to ensure appropriate and timely access to the specialty Seniors Mental Health Behaviour Support Services; navigation to appropriate specialty inpatient services; and help to coordinate access to other community health/support services. In addition, the role provides waitlist management support including regular follow-up and/or clinic activities. As an integrated team member, the Central Access and Referral Coordinator participates in profession-specific and inter-professional activities that are client-centred and contribute to quality and safety, and operational efficiency, while promoting professional and program development.

Qualifications
  • Baccalaureate degree in Nursing, preferred or an equivalent combination of education and experience
  • Current unrestricted registration with the College of Nurses of Ontario
  • Education completed or experience working in the PIECES framework
  • Gerontological Nursing OR Psychiatric/Mental Health Nursing certification, through Canadian Nurses Association; OR Gerontology Diploma preferred
  • Minimum of 3 years’ recent experience in geriatrics and/or geriatric psychiatry
  • Minimum 3 years’ experience working in a wide range of community settings including long-term care homes and clients’ homes
  • Demonstrated recent experience in working in collaboration with families and community support services
  • Experience in assessing educational needs and/or developing educational programs for colleagues, service providers, clients and family members
  • Community development experience, preferred
Position Responsibilities
  • Acts as an entry point to Providence Care Seniors Mental Health Behaviour Support and assists in the coordination of other dementia services across the South East region
  • Connects with the client/family and responds to the referral source in a timely manner
  • Identifies areas of RISK or potential RISK and implements immediate measures liaising as required with community partners, including police, primary care, and other crisis services
  • Provides intake and triage including assessment, prioritization, system navigation and facilitates consultation with the appropriate Seniors Mental Health Behavioural Support Services team member
  • Obtains and documents, as per College of Nurses of Ontario standards, an initial database of information from all appropriate sources, including an initial plan for further assessment
  • Provides support and navigation to appropriate inpatient level of services when required (e.g., Providence Care Inpatient SMH Unit; BSTU)
  • Provides referral management and waitlist management
  • Maintains a database of community health and support resources relevant to the SMH BSS population within the SE LHIN and bordering LHINs (Central East and Champlain)
  • Utilizes knowledge of community health and support services, and maintains relationships with these partners, to facilitate appropriate linkages as required
