Job Description

The Program Consultant will provide clinical assessment and treatment recommendations through a comprehensive approach. The program consultant will conduct a comprehensive assessment, engage in case management and rehabilitation support. Individualized treatment plans are developed to improve the patient's quality of life and ability to engage meaningfully with their community.

What must you have?

  • Active registration with the Ontario College of Social Workers and Social Service Workers.
  • Undergraduate Degree in Social Work and related experience.
  • Valid Ontario driver’s license and a vehicle and be able to travel throughout North Simcoe Muskoka.

What else do you bring?

  • Current clinical experience and knowledge of social work as it applies to the area of acquired brain injury.
  • Possesses excellent verbal and written communication skills.
  • Well-developed interpersonal skills.
  • Proven attendance record.
  • Demonstrates understanding and complies with patient and staff safety policies and procedures that foster a safe and inclusive environment for all.
  • Ability to perform the essential duties of the job.
  • Effectively uses empathy in interactions with others and demonstrates behaviours consistent with Mackenzie Health’s Commitment to Caring.
