Overview

LMC Pharmacy is a consultation-based pharmacy specializing in diabetes training, education, and comprehensive medication assessments. We are co-located in LMC Healthcare clinics, Canada’s largest specialist care provider in diabetes, endocrinology & metabolism. LMC Pharmacists work collaboratively with the endocrinologists and clinic team to deliver patient education, training, medication management, MedsCheck services as well as medication dispensing and medication administration services.

Our progressive team is looking for a Pharmacist to join us on a permanent, full-time basis at our LMC Vaughan location. In this position, you’ll use your clinical expertise to connect with patients living with diabetes, conducting MedsChecks, optimizing their medication therapy, and empowering them through education and support.

What You’ll Do
  • Conduct diabetes-focused medication assessments with patients to review medications, identify issues, and optimize therapy.
  • Provide personalized education on diabetes management, medication use, lifestyle strategies, and adherence.
  • Build rapport and foster trusted, ongoing patient relationships over the phone and in-person.
  • Collaborate with prescribers and other healthcare providers to ensure coordinated care.
  • Document consultations and follow-ups in compliance with standards of practice.
  • Stay current with diabetes guidelines and best practices to deliver evidence-based care.
  • Support all responsibilities related to pharmacy operations including but not limited to medication/supplies dispensing and administration
What We’re Looking For
  • Interest in helping people with diabetes improve their health and wellbeing
  • OCP Part A licensed pharmacist in good standing
  • Certified Diabetes Educator (CDE) certification preferred
  • OCP Injection Certification – current certification and first aid
  • 2-3 years of Ontario pharmacy practice and diabetes education experience preferred
  • Must have previous retail pharmacy experience
  • Kroll proficiency and EMR experience an asset
  • Excellent verbal and written communication skills
  • Excellent interpersonal, organizational and time management skills
  • Ability to travel locally to other LMC sites
