Job Summary: The Personal Support Worker (PSW) provides personal support and homemaking services to clients and families within VON policies, procedures, and standards. The PSW collaborates with the healthcare team to meet clients' needs, promoting independence and wellbeing. This position is eligible for a $3.18/hour wage enhancement from the Ontario Government for hours spent caring for clients.

Key Responsibilities:

  • Assess and evaluate clients' and families' home and personal situations to update care plans accordingly, informing the supervisor of any changes.
  • Assist in developing personalized support care plans under the guidance of the Program Manager, incorporating information from funders, managers, and clients/families.
  • Implement care plan changes for safety, liability, and financial reasons, reporting any significant changes to the supervisor or manager.
  • Perform personal support tasks tailored to clients' needs, including light cleaning, meal prep, clothing care, shopping, and household safety monitoring.
  • Provide personal care such as assistance with mobility, hygiene, dressing, and feeding, as directed by the manager or supervisor.
  • Offer bedside care when required, under nurse supervision.
  • Deliver respite care and support to clients' families, including child care to provide family relief.
  • Organize daily activities, set priorities, and ensure efficient service delivery under supervision.
  • Complete daily checks and documentation to maintain service quality and efficiency.
  • Assess clients’ communication and relational abilities.
  • Communicate with the healthcare team regarding clients’ physical, emotional, and homemaking needs, reporting concerns promptly.
  • Comply with Occupational Health and Safety legislation, VON policies, and risk management practices to ensure client safety.
  • Perform other duties as assigned.

External and Internal Relationships:

  • Maintain regular communication with supervisors and Program Managers regarding client care plans and other relevant updates.

Education, Designations, and Experience:

  • High school diploma; secondary education preferred.
  • CCA Certification required in Nova Scotia.
  • Certificate in Home Support Worker (Level I, II, III), Personal Support Worker program, or Health Care Aide.
  • Current Standard First Aid or CPR Level C Certificate, if applicable.
  • Experience in household management and working with elderly, ill individuals, or children.

  • Satisfactory Criminal Records Check; Vulnerable Sector Search and/or Child Abuse Registry Check may be required.
  • Availability for flexible hours.
  • Use of Personal Protective Equipment (PPE) as required.
