Salary Range: $69.46 - $81.71 /hour

Responsibilities
- Work within a collaborative practice model with four family physicians and a comprehensive interdisciplinary team to support a geriatric population requiring long-term care within four units (100 beds) in the Veterans Care Program.
- Engage in role-related teaching and research activities in addition to clinical duties where applicable.
Essential Qualifications
- Valid registration with College of Nurses of Ontario as a Registered Nurse Extended Class with no restrictions
- Master's of Science in Nursing (MScN) or Master’s of Nursing (MN)
- Primary or Acute Care Nurse Practitioner certificate
- Member of RNAO, NPAO, Canadian Nurses Protective Society (CNPS) with professional liability insurance
- Knowledge of a Safety Culture in a Health Care Setting in compliance with the OHSA
- Ability and commitment to act as a supervisor under OHSA
- Advanced knowledge and skills with a multi-system focus in physical assessment, consultative support and evaluation specific to the care of geriatric patients
- Superior interpersonal, coaching and communication skills to effectively work within a multi-disciplinary team setting
- Demonstrated critical thinking and problem-solving skills
- Ability to demonstrate a facilitative teaching approach with patients/families and staff
- Demonstrated experience in conducting and/or promoting research use
- Experience with supporting change
- Demonstrated leadership experience
- Comprehensive knowledge of and ability to mobilize community resources
- Demonstrated experience in mental health and psychogeriatric assessments
Preferred Qualifications
- Proficiency in French would be an asset
- Demonstrated ability to collaborate with the interdisciplinary team to develop and implement patient/resident-centered care delivery practices
- Demonstrated experience promoting evidence-based practice
- Demonstrated advanced skills in physical assessment
- CNA Certification in Gerontological Nursing
- CNA Certification in Hospice/Palliative Care Nursing

Immunization Requirements
