Job description

JOB SUMMARY:

The Licensed Practical Nurse will report directly to the Health Director and will work closely with the Nurse-in-Charge/Registered Nurse on site. The Licensed Practical Nurse will adhere to and follow the Whitefish Lake First Nation #128 Employee and Financial Policy.

The Licensed Practical Nurse delivers basic nursing care, monitors patient health, assists with daily living activities, and maintains accurate medical documentation. The role supports patient safety, comfort and wellbeing. Promoting safety, comfort, ensuring that medication administration, wound care, chronic disease management, vital‑sign monitoring, and patient education is delivered to Whitefish Lake First Nation #128 members to ensure optimal health outcomes.

DUTIES:

Provide nursing care to clients in their homes using the care plan prepared by the registered nurse (RN) or Nurse‑in‑Charge.

Provide one‑to‑one teaching to clients/families/caregivers.

Performing assessments including vital signs, monitoring symptoms, and reporting changes in condition to the RN or Nurse‑in‑charge.

Administer medications safely via oral, topical, inhaled, subcutaneous, and intramuscular routes.

Manage chronic conditions such as diabetes, hypertension, COPD, and mobility limitations.

Assist with ADLs (bathing, grooming, feeding, toileting, mobility when needed)

Take the time to foster relationship building at each encounter as a method to build healthy connections with the client and their caregivers. Building trust may take time as the person receiving care may have a mistrust of western systems and processes for many reasons, including colonization, system racism, racism in healthcare and past of ongoing traumas.

Practice is consistent with approaches that foster cultural safety and trauma‑informed care.

Care plans may by created by the licensed practical nurse (LPN) under the supervision of an RN or Nurse‑in‑charge.

Evaluate the client’s progress toward the goals of the care plan.

The LPN will follow the care plan for each treatment and prescribed procedure.

Document care provided and any observations of the client’s change in status.

Maintain client records according to policy and guidelines.

May be required to work overtime and weekends from time to time.

Perform the personal care treatments and procedures for assigned home care clients under the following conditions:

• The LPN is required to continuously exercise judgement.
• Make decisions within the parameters and the scope of practice of their profession.
• Participate in staff meetings, case reviews, and in‑service programs as requested and execute assignments as required by the Registered Nurse or Nurse‑in‑Charge with scope of work.
• Maintain client confidentiality.
• Performs other duties as assigned by the Nurse in Charge

Accountability

Report/document any concerns regarding a client’s condition to the Nurse in Charge in a timely manner.

Report on work‑related concerns to the Supervisorsubmit necessary home care reports.

Complete all reporting in a timely manner as required by Nurse in charge.

Must follow all work processes (financial, reporting, clinical, and safety practices) established by employer.

The LPN will work under the direction of the Nurse in Charge in collaboration with the Health Director.

Must be a team player within the Home Care program along with the Health Centre Programs.

All and any other duties called on by the Nurse‑in‑charge within the scope of the CLPNA Standards of Practice and Code of Ethics document.

Whitefish Lake First Nation #128 requires and expects the LPN to work within their scope of practice as outlined in the CLPNA Standards of Practice and Code of Ethics document and within the Restricted Activities and Advance Authorization policy and will abide by the Practice and Education Requirements for Restricted Activities and Advanced Practice. The Whitefish Lake Band #128 fully supports and endorses all CLPNA Standards of Practice and Code of Ethics Policy and Standards to govern the practice of the Licensed Practical Nurse.

QUALIFICATIONS AND SKILLS:

Graduation from a recognized practical nurse program

Member of good standing with current licensure with the College of license Practical Nurses of Alberta (CLPNA).

Must provide proof of registration through the CLPNA.

Specific Clinical Training/Certification:

• Current cardiopulmonary Resuscitation (CPR)
• Health Care provider and First Aid certificate
• Inoculant certification – First Nations and Inuit Health Branch
• Trained in the use of InterRAI Home Card Assessment System and program.
• Demonstrated fluency of database and clinical computer programs used for managing client eFiles. (e.g., CHIP, CARE, Inter‑RAI, Netcare)
• First Nation cultural and traditional knowledge
• Community based naloxone certification
• Wound care/foot care education or certification
• Palliative care education or certificate
• Transportation of Dangerous Goods (TDG) certificate
• Workplace Hazardous Materials Information System (WHIMIS)
• Class 5 license
• Computer skills/technology
• Criminal Record check/vulnerable sector check
• Case management course – for initial client assessments or case management

WORKING CONDITIONS:

Physical Demands - The Licensed Practical Nurse will be in regular contact with the public; verbal communications are conducted face to face. Will be on the switch board, computer, and assignment will include the occasional evenings and/or weekends. Will be filing and is required to post on social media. Services are delivered to clients’ s home, Health Centre, and other designated areas.

Environmental Conditions - The Licensed Practical Nurse will be working in a busy and occasionally noisy environment. There may be a number of activities and situations happening at once and the demand for the Worker's attention to multi‑tasking. Worker will be travelling in challenging weather and road conditions and will also be in a non‑standard workplace which could pose challenges (pets, etc.)

Sensory Demands - Sensory demands include use of the computer, which may cause eyestrain and occasional headaches. May experience smells of bodily fluids and witness blood trauma related injuries/incidences from time to time.

Mental Demands -There may be a number of situations happening at once and the Licensed Practical Nurse must be prepared to handle incidents, trauma related matters, and emergencies at any time.
