As a Licensed Practical Nurse with Kindred Home Care, your role will be to work directly in our clients’ homes and provide professional, patient-centered care and truly make a difference in our clients’ lives:

• Elevate Health Outcomes: Your direct, one-on-one care, including comprehensive assessments, medication management, and specialized procedures, will ensure our clients receive the focused attention they need to maintain and improve their health at home.
• Cultivate Trusting Connections: By working closely with clients and their families, you will build meaningful relationships, becoming a consistent and reliable healthcare partner in their journey.
• Promote Independence and Dignity: Through personalized care and assistance, you will help clients manage chronic conditions and daily needs, empowering them to live with greater autonomy in the comfort of their own homes.
• Ensure Seamless Care Coordination: Your accurate documentation and collaboration with the RN, interdisciplinary team, and physician will ensure a cohesive and effective care plan for every client.
