Job summary

A healthcare solutions provider is looking for a compassionate Nurse Practitioner to conduct in-home wellness assessments for Medicare members in Hamilton, Ontario. This position offers flexibility and requires performing comprehensive assessments and building relationships with patients. The ideal candidate should hold a valid NP license and have prior patient care experience. This is a 1099 (PRN) role allowing for a minimum commitment of 30 hours monthly while delivering quality care to at-risk populations.

Qualifications

• Must have a valid unencumbered NP License for the state you will be working in.
• 3 years of patient care experience preferred (primary care/adult/geriatric, EMR).
• Access to reliable transportation that enables travel to members' homes.

Responsibilities

• Perform annual in-home wellness visits and risk adjustment health assessments.
• Build relationships of trust with members through exceptional communication and empathy.
• Deliver patient health education on topics such as pain management and medication.
