Qualifications

• Current College of Nurses of Ontario (CNO) registration in good standing.
• CPR Certification.
• Current immunization record including 2 step TB test.

Responsibilities

• Develop and maintain effective Care Plans with respect to Community Health Services.
• Provide exceptional nursing services both in-person and virtually.
• Liaise with patients and interdisciplinary care team members.
