Un fournisseur de soins de santé communautaire recherche un coordonnateur des soins. Ce rôle consiste à gérer efficacement les aiguillages et à offrir un service à la clientèle de haute qualité. Le candidat idéal a au moins 2 ans d'expérience dans un rôle similaire, une formation en administration de bureau médical et d'excellentes compétences en communication. Ce poste à temps plein est basé à Toronto, avec un taux horaire de 20 à 24,10 CAD.

Le coordonnateur des soins, qui relève directement de son chef d'équipe, travaillera en étroite collaboration avec les sources d'aiguillage, les cliniques et les autres intervenants, au besoin, afin de gérer les aiguillages au moyen d'un processus efficace et efficient qui donne lieu à un accès approprié aux soins. Le coordonnateur des soins est responsable de fournir un service à la clientèle de haute qualité qui dépasse les normes et les attentes de CBI Health et des intervenants et sera responsable d'une gestion optimale des références.
