Overview
CareRx is looking for a Clinical Consultant Pharmacist to join our Vancouver Island team. The Clinical Consultant Pharmacist is responsible for maintaining continuity of care for clientele through collaboration between onsite physicians, nursing staff, and other health providers at long term care, supportive living, and other congregate living communities. The role includes completing clinical pharmacy activities in alignment with the Continuing Care Health Service Standards, site policies, internal CareRx policies, and the Ministerial Order for Clinical Pharmacy Services.
Responsibilities
- Act as a liaison between long term care homes and retirement communities and CareRx Pharmacy to ensure optimal resident care, pharmacy services, and clinical support
- Travel to LTC homes and retirement communities
- Build and maintain relationships within the community to ensure ongoing collaboration and communication between home/community leadership teams and pharmacy teams to develop/maintain strong partnerships
- Complete medication reviews in currently serviced retirement communities and long-term care homes serviced by CareRx Pharmacy
- Complete all required documentation and billing processes as required by MOHLTC, College of Pharmacists of British Columbia, and other federal and provincial standards and regulations
- Provide education to residents and staff on various health-related topics including the MedsCheck program and attend community-based wellness fairs and other events as required
- Conduct education sessions within Long Term Care Homes and Retirement Communities within scope of practice, as applicable (e.g. policy and procedure, web portal, eMAR, influenza protocol, etc.)
- Support CareRx Pharmacy policies and procedures within retirement communities and long-term care homes
- Complete medication audits on-site in LTC Homes and Retirement Communities as required
- Ensure regular on-site visits are made to all assigned clients
- Attend/participate in annual professional conferences and trade shows, when applicable
- Participate in the development and implementation of industry-leading clinical pharmacy service, when applicable
- Be an education, policy and pharmacy initiative resource
- Be available for special clinical team projects as required
- Complete/collaborate in the develop of medication audit follow-up improvement plans when applicable
- Works collaboratively with other team members to support applicable homes and communities
- Participate in pharmacy-related transition processes (e.g., new client transitions, eMAR transitions, etc.)
- Help with pharmacy operations/drug distribution when required
- Attend and actively participate in clinical team meetings
What You Will Bring To The Team
- Insured and currently licensed in good standing with the College of Pharmacists of British Columbia.
- Proven ability to foster strong partnerships with other members of the healthcare team
- Effective organization, planning, and time management skills
- Knowledge of Kroll Windows application and Microsoft Office programs
- Complete understanding of provincial regulations governing LTC homes and Retirement Communities
- Must be familiar with the provincial standards of practice established for LTC pharmacies
- Excellent customer service and interpersonal skills, oral and written communication skills and be able to interact with persons of varying levels of professional expertise
- Excellent time management skills
- Skilled in critical thought processes and be able to prioritize appropriately
- Must demonstrate initiative and creativity
- Self-directed requiring minimal supervision and able to work independently
- Motivated to achieve results that reflect a high standard of quality
- Good listening skills and be adaptable to changes in job environment

Compensation
Compensation Range: $109,200-$119,600 annually (based on a 37.5 hour workweek). Actual pay rate will be based on the individual candidate’s credentials and experience.
Location
Location will primarily be LTC homes and retirement communities in Vancouver Island, preferably on the eastern coasts of the Island.
Work Shifts
This is a full-time position with flexibility and opportunity to work-from-home and on-site from the care homes. Work schedule is focused within the standard Monday to Friday 9am-5pm.
