What You Will Do
- Conducting comprehensive nursing assessments and developing customized care plans.
- Recognizing and analyzing assessment findings to prioritize nursing care and interventions.
- Administering and managing infusion therapy, including medication administration and fluid management.
- Inserting and maintaining peripheral and central venous access devices (VADs), including dressing changes and line maintenance.
- Medication management and promoting adherence to the patient’s medication regimen.
- Providing wound care, post-surgical care, and drain care including comprehensive assessments and dressing changes.
- Implementing infection control measures to prevent the transmission of pathogens.
- Assessing the need for additional home supports and referrals.
- Collaborating with an interdisciplinary team, including physicians, nurse practitioners, and other healthcare professionals, to optimize patient well-being and health outcomes.
- Educating patients and families on chronic disease management, potential complications, medications, and self-management strategies.
- Trouble-shooting clinical issues and implementing effective solutions/strategies to reduce risks.
- Appropriately identifying and addressing clinical issues or stabilization opportunities seamlessly.
- Documenting assessment, interventions, and evaluation of patient progress.
- Delivering evidence-based nursing care that promotes safe and effective recovery and healing.
What You Will Need To Be Successful
- Registered Licensed Practical Nurse or Registered Nurse with the College of Registered Nurses of Alberta or College of Licensed Practical Nurses of Alberta, in good standing.
- Minimum 2 years of acute care experience (ICU, ED, medical-surgical or equivalent) preferred.
- Previous home care/ community nursing experience preferred.
- A clear criminal record check with vulnerable sector screening.
- Proof of negative TB test and professional liability insurance.
- Valid CPR Certification.
- Strong critical thinking, nursing assessment and decision-making skills.
- Strong verbal and written English communication skills, bilingual in French an asset.
- Ability to travel independently within the community – a valid driver’s license, vehicle and vehicle insurance required.
- Proficiency in IV therapy, oxygen therapy, wound and drain care, and advanced nursing assessment.
- A commitment to continuing education and willingness to actively participate in departmental educational activities.
- Proficiency with various healthcare technology (e.g. point of care testing, mobile apps, remote patient monitoring).
- A commitment to evidence-informed practice and quality service delivery.
- Ability to work collaboratively with an interdisciplinary health care team.
- A professional and respectful attitude to patients, families, the public and colleagues.
