Qualifications

• Bachelor's degree in Social Work, Sociology, Psychology, or a related field.
• 3-5 years of experience in social work, case management, or care coordination.
• Strong understanding of social care, client advocacy, and resource navigation.
• Excellent communication and interpersonal skills.

Responsibilities

• Conduct comprehensive needs assessments via video calls and phone consultations.
• Develop personalized social care plans with clients, families, and stakeholders.
• Coordinate and schedule services with a network of providers and agencies.
• Monitor progress and adjust care plans as needed.
• Provide information and referrals to clients regarding services and resources.
• Maintain confidential client records and case notes in a digital system.
• Serve as primary point of contact for clients and address concerns.
